Your rights
When it comes to your health information, you have certain rights. You may:
- inspect or obtain an electronic or paper copy of your medical record and other health information we maintain about you, subject to limited exceptions;
- ask us to correct health information you believe is incorrect or incomplete;
- request confidential communications in a particular way or at a particular location;
- ask us to limit what we use or disclose for treatment, payment, or healthcare operations, although we are not always required to agree;
- request an accounting of certain disclosures made during the prior six years;
- obtain a paper copy of this notice;
- choose someone with legal authority to act on your behalf; and
- complain if you believe your privacy rights have been violated.
If you pay in full out of pocket for a service and ask us not to disclose information about that service to a health plan for payment or healthcare operations, we will comply unless disclosure is required by law.
Your choices
For certain health information, you may tell us your preferences about sharing. These situations may include communication with family, close friends, or others involved in your care; disaster-relief efforts; fundraising; marketing; sale of information; and certain uses of psychotherapy notes. We will follow your instructions when the law gives you the right to decide.
How we may use and disclose health information
Treatment
We may use and share your information to provide, coordinate, and manage treatment, including communication with pharmacies and other healthcare professionals involved in your care.
Payment
We may use and disclose information to bill and receive payment for services, process transactions, provide receipts, or support reimbursement documentation you request.
Healthcare operations
We may use and disclose information to operate the practice, improve care, conduct quality and compliance activities, train personnel, manage risk, and perform administrative functions.
Other permitted or required uses
As allowed or required by law, we may use or disclose information for public-health and safety activities; reporting suspected abuse, neglect, or domestic violence; health oversight; judicial or administrative proceedings; law-enforcement purposes; workers’ compensation; organ and tissue donation; coroners, medical examiners, and funeral directors; certain research; national-security activities; and to prevent or reduce a serious and imminent threat to health or safety.
Uses requiring authorization
Uses and disclosures not otherwise permitted by law generally require your written authorization. You may revoke an authorization in writing, except to the extent we have already relied on it. We generally will not use or disclose psychotherapy notes, sell protected health information, or use protected health information for marketing without authorization when authorization is required.
Records relating to substance use disorder treatment may receive additional protection under federal law. When those rules apply, we will use and disclose such records only as permitted by law and any applicable authorization.

- We will notify you as required if a breach may have compromised the privacy or security of your information.
- We must follow the duties and privacy practices described in the notice currently in effect.
- We will not retaliate against you for filing a complaint or exercising a privacy right.
