When this right applies
Under federal law, healthcare providers generally must give patients who do not have insurance, or who are not using insurance, an estimate of expected charges for non-emergency healthcare items and services.
Mind Wellness is a private-pay practice. Current standard appointment prices are displayed on the website before booking. Your written Good Faith Estimate may include additional information based on the expected frequency and duration of care.
When you should receive an estimate
- If an appointment is scheduled 3–9 business days in advance, the estimate generally must be provided within 1 business day after scheduling.
- If an appointment is scheduled 10 or more business days in advance, the estimate generally must be provided within 3 business days after scheduling.
- If you request an estimate before scheduling, it generally must be provided within 3 business days after the request.
You may request a Good Faith Estimate at any time by contacting Mind Wellness.
What the estimate means
A Good Faith Estimate is not a contract and does not require you to receive care. It reflects items and services reasonably expected at the time it is prepared. Your treatment needs may change, and services that could not reasonably have been anticipated may not appear on the original estimate.
If your bill is substantially higher
Keep a copy of your estimate. If a bill from a provider is at least $400 more than that provider’s Good Faith Estimate, you may be eligible to use the federal patient-provider dispute resolution process.
For questions about these rights or the dispute process, visit CMS Medical Bill Rights or call the No Surprises Help Desk at 1-800-985-3059.
