What these sessions are for
Follow-up appointments are for patients who have already established care and are continuing their treatment journey. These sessions provide a space to monitor progress, adjust treatment plans as needed, and address any new or ongoing symptoms.
Whether you're responding well to your current plan or navigating new challenges, follow-ups allow for ongoing support and collaboration. This may include evaluating the effects of medication, discussing coping strategies, exploring therapy goals, or checking in on how daily functioning has been impacted.
The goal of each follow-up is to ensure your care remains aligned with your needs and to help you continue moving toward stability, insight, and improved well-being.
Why follow-up are important
Mental health isn't something you solve in one session. Ongoing mental health care with the same provider(s) allows for consistency and continuity with people who already understand your story.Instead of repeating your history at every visit, you can build on the foundation we've already created-saving time and deepening care over time.
How we track progress over time
Everyone's experience with mental health is different-so we don't rely on a single metric to measure progress. In follow-up sessions, we consider a mix of factors: symptom changes, sleep and energy patterns, emotional resilience, and how you're functioning day to day.
You might not notice big shifts immediately, and that's okay. We're looking for signs that things are moving in the right direction-even in small ways. These check-ins help us fine-tune your plan so it stays realistic and aligned with your goals, whether that's feeling more stable, managing stress better, or returning to things you enjoy.
